Parents,

Flu and Cold season is approaching quickly.

Please do not send your child to school if:

  • They have had a fever within the last 24 hours without giving fever reducers (Tylenol).
  • They have had vomiting or diarrhea within the past 24 hours without giving anti-nausea medication.
  • They have had a rash in the past 24 hours.
  • They have had Pink eye in the last 24 hours. Must be clear for 24 hours or has started drops with doctor's excuse.

Your student will be sent home and cannot come back for 24 hours.

The School Medication Prescriber/Parent Authorization form is used for ANY student that would need medication during school hours. This form is used for Prescription medication and over the counter medication.
 

If your student needs an over-the-counter medication (Tylenol, Tums, Benadryl, etc.) they must have this form completed. The parent must bring an unopened container with the medication to the school. Students are not allowed to carry medication, unless it is an inhaler or Epi-Pen, and they have permission.
